As a member of the Interdisciplinary Group, the Home Health Aide provides personal care services to hospice patients. The Home Health Aide performs related tasks in the home and nursing facility settings which are in accordance with the Plan of Care, and are assigned by the RN Case Manager.
Minimum Knowledge, Skills and Abilities
1. Successfully completed a nurse assistant/Home Health Aide certification course or is in progress to obtain HHA certification.
2. Ability to make meaningful observations and write concise, accurate reports.
Ability to recognize the needs of people and to maintain good relationships in dealing with them. CPR certified.
4. Previous experience as a home health aide.
5. High school graduate.
6. Effective oral and written communication skills.
Normal patient care environment with little exposure to noise, dust, temperature, and the like.
Very frequent exposure to moderately adverse working conditions due to performance of certain distasteful patient care activities.
Ability to concentrate and pay close attention to detail fifty percent or more of work time.
Ability to stand, bend and walk more than eight percent of work time.
A medium-heavy physical workload demand based on a CHP Industrial Rehab. Standard Work Classification Table adapted from the US DOT Dictionary of Occupational Titles
Directly accountable to the RN Case Manager and/or Clinical Manager.
