The Case Manager is a registered nurse who applies nursing knowledge and clinical skills using the nursing process to ensure quality and safe delivery of home care services. The Case Manager possesses strong clinical skills and a knowledge base in a specialized area and utilizes leadership skills in coordinating and/or providing home care services for a group of patients within that specialty area. These services include education of patients/families and staff, collaboration with the physician in the plan of care, total coordination of patient care and the quality monitoring of appropriateness of patient care. The group of patients can include home care patients of all ages. The Case Manager facilitates achievement of outcomes.
The Case Manager is a registered nurse with a minimum of 2 years recent acute care or home care experience.
Graduate from accredited School of Nursing
Current RN license in State of Ohio
Must possess BSN or obtain within 5 years of employment
Orientation program is provided
BLS certified by American Heart Association
Performs repetitive tasks/motions; Hears alarm bells, telephone, and other sounds; Hears normal conversation; Have good manual dexterity; Have good eye-hand-foot coordination; Have clarity of vision: Near (< 20”); Have clarity of vision: Mid (> 20” – 20’); Distinguish colors
