Must possess level of knowledge of counseling skills and services, hold a Master's degree in Social Work from an accredited graduate social work program accredited by the council on Social Work Education.

The Social Worker/Counselor  leads the planning and coordination of patient care. Responsible for providing social services for patients and families of inpatient behavioral health as part of a multidisciplinary team with emphasis on group and individual therapy. Works closely with psychiatrists, as an active member of the Treatment Team by attending Care Plan meetings. Responsible for psychotherapy group, wellness groups, psychosocial assessments and faciliting individual, marital and/or family therapy. Develops, implements, reviews and updates treatment plans as well as case management and discharge planning. Ensures assessments are completed within regulatory timeframe. Has ability to perform all requirements of the social worker/ counselor I. Leads weekly clinical supervision meetings as assigned. Designs and implements specialized therapeutic groups for patients and families in all program areas, including education, support services and community resources.

Must provide individual assessment and group treatment in both inpatient and outpatient setting, provide assistance with discharge planning, assessments and psychoeducation groups, and perform Intake/Clinical Assessments in the ED and PHP/IOP for scheduled assessments/walk-ins, or on the medical floors.

The Social Worker/Counselor II documents in medical record in accordance with accepted policy in a timely manner. A licensed independent chemical dependency counselor or licensed independent social worker may be asked to provide supervision to staff eligible and pursuing this licensure.

Holds others accountable to standards of performance, and displays a curiosity and desire for knowledge and staying current with colleagues, being curious about business and the health care services industry. He or she will regularly invests time in encouraging the team to innovate, continuously improve processes and share knowledge. Performs clinical supervision is assigned.

Family Sessions and/or meetings to promote complaince with treatment. Quality Treatment Outcome and Pateint Experience data collection as assigned.
