Job Summary:

Provides evaluation, reassessment, and treatment of patients referred for home health occupational therapy and supervises the plan of care and the clinical work of the COTAs assisting in the care. Serves as the home health case manager on therapy only patients taking responsibility for leading the care team, managing the total care plan and assuring adherence to regulatory and agency guidelines for those cases.

Job Functions:

A. Completes accurate evaluations, reassessments and treatments of occupational therapy patients

B. Oversees COTA treatments assuring adherence to the plan of care and quality outcomes

C. Develops patient plan of care and adapts the plan according to assessment data and the patient's needs.

D. Schedules work independently and according to patient's clinical need and agency policy

E. Documents care, progress, goals, contacts, and orders according to policy and regulation.

F. Serves as a care team member, communicating effectively with patients, families, caregivers, physicians, and third party payors.

G. Works autonomously without direct supervision in the field.

H. Uses computerized technology for clinical care effectively after on the job training

I. Provides appropriate patient and caregiver teaching

J. Serves as primary case manager on therapy only cases: 1. Leads teams in working toward patient goals for achievement of highest function possible, quality of life improvement, and community re-integration if possible. 2. Leads team effectively including holding case conferences 3. Assures all team member's documentation is complete, timely and meets standards 4. Assures patient and caregiver teaching, equipment needs, and home modifications are addressed relevant to the scope of service provided. 5. Is competent in following guidelines for different payors and contracts and communicates effectively with them 6. Refers patients to appropriate resources including identification of need for other professional/agency intervention 7. Initiates discharge planning at onset of care based on patient's needs and modifies the plan of care as appropriate

K. At discharge, assures coordination with any needed ongoing services and completion of discharge teaching.

Required:

Education Graduate of AOTA accredited OT program

Licensure / Certification Kentucky licensed and Illinois license eligible
