SUMMARY OF JOB RESPONSIBILITIES
Responsible for managing all aspects of the patient/client intake process including establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes. The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of Hospice of the Valley.
Is a graduate of an accredited school of professional (registered) nursing. BSN preferred.
Is currently licensed as an RN through the Ohio Board of Nursing.
Has at least three to five years experience in home care/hospice preferred. Pain management and IV skills required.
Has knowledge of corporate business management, governmental regulations, CHAP standards, and private payor practices.
Demonstrates good communication skills, negotiation skills, and public relations skills. Has excellent observation, good nursing judgment and communication skills. Ability to provide good written documentation in a timely manner.
Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
Possesses and maintains current CPR.
Has a valid driver’s license and is insured in accordance with state and/or organizational requirements.
RESPONSIBILITIES AND DUTIES
Directs all daily patient/client referral, intake operations and implementation of intake policies.
Ensures compliance with all referral/intake regulatory requirements.
Enters referral/clinical data into Allscripts software system and gathers necessary information from various sources (EPIC, existing medical record sources, etc).
Directs the implementation of improved work methods and procedures to ensure patients/clients are admitted in accordance with policy. Coordinating both inpatient and outpatient hospice services.
Ensures maximum third party reimbursement through direct oversight of insurance verification and authorization processes.
Provides professional nursing care by utilizing all elements of nursing process.
Initiates and sustains as needed the implementation of orders for medications and treatments as prescribed by the physician in the physician’s plan of care. Initiates and applies appropriate preventative and therapeutic nursing procedures and techniques and maintains communication regarding patient care with the physician and supervisory personnel.
Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.
Assesses and evaluates patient’s status by writing and initiating plan of care. Completes admission assessment and documentation within the Hospice policy. Works with IDT in formation of initial care plan.
Teaches the patient/family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients/families/caregivers. Works in concert with the other Hospice interdisciplinary team members.
Provides and maintains a safe environment for the patient.
Works in cooperation with the family/caregiver and hospice interdisciplinary team members to meet the emotional needs of the patient/family/caregiver.
Accepts responsibility of assignment to perform a specialized procedure, such as IV Therapy, only when qualified with specialty training, proven competency, clinical background, and expertise in the specialty area.
Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes.
Actively participates in quality management program.
Accepts responsibility to complete admissions after hours as needed.
Performs other duties as assigned.

Hospice personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, hospice personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All personnel must attend mandatory educational programs.
